What Is the CEHR Patient Portal?
The Community Electronic Health Record (CEHR) is MCMHA’s Patient Portal. The CEHR is a way to see your personal health information, see upcoming appointments, and view and sign some documents. It is private, secure, and confidential. Ask your case manager at your next appointment for assistance registering for your account.
How to Register
You will need your CRCT ID, unique pin, and a valid email address to register. Your case manager will provide you with your CRCT ID, unique pin, and print out instructions to register for your account.
